(3 of 3) Severe clean-up in usock.c and other places.

Refactoring lots of repeated code, a couple minor bugfixes that could cause a
crash and some formatting changes but no significant functional changes.

Please apply the patches in the same order.  Please test first and if there are
any issues please report, especially Thomas please see if this is enough for
bug #1028.  I couldn't find a SIM card for testing the pin requests today.

The three patches don't implement lgsm_pin_status() because it's not strictly
necessary for PhoneKit, but it can be implemented on top of this if anyone has
some time.
