3G sim cards

Can you tell me the numbers on the chip side of the SIM?
Not the 20 digit ICCID but the 5 digit SKU followed by a letter and then 
the 4 digit Vendor and Version number underneath it.

For example G&D cards say
    71234 D
    2202
