dial prefix string before sending number entered by user

Hi,
   This is a good example of what the low cost calling extension is 
designed to handle.  There's not a lot of detail up on the wiki at the 
moment but there is a mention in the wish list and I'll be padding out 
the information on how this works as/when I get some time to put it 
together.
