Workaround for failed video init

Should work around the failed video init of register 0x40 that is seen
sometimes.  Wrong value (0x0000) in register 0x40 means PLL1 cannot lock; the
VCO issues the lowest frequency of 1.7MHz and the LOCK bit is clear.  After
performing the init, this patch checks that both PLLs are in fact in lock, if
either or both are not, it repeats the init.  Since only register 0x40 is
affected it seems to be some timing issue to do with the GPU and CPU access to
the register.
