[patch] passing errors back to the user & knowing when received the
final response

Hi
Currently if there is an error during processing the at response the
user is left waiting in many cases.
This patch ensures the user always receives a response for a given request.
The patch introduces a free list for a small tune-able number of ucmd
(gsmd_msg_hdr) headers that can be used to relay back errors to the
user, including no memory conditions.
These changes also ensures the client knows when it has received the
final response for the particular request.
This patch supersedes the single confirm patch
