[patch] single confirm (phonebook read range)

Hi

Attached patch is for a single confirm instead of multiple confirms
for a request.

The code has been applied to the phonebook read range command, to
ensure a single confirm is sent once the phonebook cache is fully
populated.

Regarding stability issues with gsmd, the keep alive timer has also
been removed in this patch since (in the current form in the openmoko
svn) the timer fires without taking into account what actions gsmd is
engaged in. With keep alive removed I have run gsmd for long periods
of testing with no problems.

Patch generated in comparison to version on openmoko svn 27th October
