That can be solved simple. If a call is active, use normal screen orientation Else, rotate it 180 deg. As I understood the problem with the speaker placement was when you hold the phone in your hand and look at the screen (video play- back, etc.) The screen orientation does not matter if you cannot see it (e.g. if you hold it to your ear).
