On Ubuntu 8.04, I found the solution in
/etc/udev/rules.d/70-persistent-net.rules where the following line needed to
be changed to usb0 at the end ( with correct MAC, of course ):

 # USB device 0x1457:0x5122 (cdc_ether)
SUBSYSTEM=="net", ACTION=="add", DRIVERS=="?*",
ATTR{address}=="00:1f:11:01:05:80", ATTR{type}=="1", KERNEL=="eth*",
NAME="usb0"

After udev restart, I got back usb0 on FR insertion
