I missed control 1 (PCM Volume) which will need to be >0 for you to hear 
anything.

If you want to hear it in the local handset you'll have to make sure it's 
routed to the speaker or earpiece as well as to the GSM module. Set controls 
81 and 85 to true for this (left mixer left playback switch, right mixer right 
playback switch)

I might have missed something else too...
